Targeted Case Management, essential to moving forward...
Targeted case management (TCM) is a Medicaid-funded service in Kentucky that helps clients with
substance use disorders (SUDs) access resources to recover. TCM services can include:
Assessment: Evaluating a client’s medical, social, and functional status, and identifying their needs and strengths
Care coordination: Involving external resources, such as family, friends, or community members, to help the client
Service arrangements: Helping the client find a provider to deliver services, such as medical, social, or educational support
Advocacy: Working with the client and their family to navigate mental health care and advocate for their needs
Recovery planning: Monitoring progress toward recovery goals
Targeted Case Management (TCM) services are tailored to each client’s specific needs and are reviewed
on an ongoing basis. TCM has been found to be effective for helping clients with SUDs stay in treatment
and recovery by allowing them to focus on their SUD treatment while also addressing other needs.

helping individuals overcome the barriers they will face because we have faced them ourselves
